SUMMARY: The AHD Stroke Coordinator is responsible for the planning, organizing, and evaluation of patient care within the Stroke Program, as well as compliance with regulatory bodies and national guidelines for stroke management. The AHD Stroke Coordinator co-leads the Stroke Team and is responsible for maintaining a strong component of patient/family advocacy with day-to-day review and coordination of multidisciplinary services. The AHD Stroke Coordinator coordinates the collection of stroke data, maintains statistical information for review and coordinates tracking of stroke patients throughout the hospital. The AHD Stroke Coordinator coordinates stroke educational programs for hospital staff, patients and the community. Performs related duties as required.
DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.
1. In collaboration with the Medical Director of the Stroke Program and Nursing Administration, maintains all necessary standards required for ongoing certification of the Stroke Program.
2. Collaborates with the Medical Director of the Stroke Program in guidance of the Stroke committees, teams and work groups. Chairs all program-related committees.
3. Serves as subject matter expert for the recommendations of the Joint Commission, Brain Attack Coalition, and the various American Stroke Association (ASA) Physician Practice Guidelines. Maintains knowledge of evidence-based practice changes, new research, and new recommendations from the Joint Commission and the American Stroke Association.
4. Provides internal and external consultation as a resource for education, document development assessment and treatment of stroke patients. Oversees regular seminars, conferences and in-services for nursing staff regarding competency and compliance to standards outlined by the Joint Commission, American Stroke Association, American Heart Association, and Brain Attack Coalition in the nursing care of patients who have suffered a stroke.
5. Oversees and implements educational programs for the Emergency Care Center and Intensive Care Unit in best practice stroke care. This includes the needs of the stroke patient who arrives within the treatment window, including TPA delivery, post TPA monitoring and management of real and potential complications. Oversees and implements training for ancillary department employees in the specific expectations of primary stroke centers (i.e. turnaround time for results of CT scan, laboratory results, EKG and door-to-needle times.) Oversees the education of members of the EMS system, including paramedics and dispatch/radio nurses.
6. Assists in the development and coordination of patient education. Plans and collaborates the development of an annual community outreach event for public education of stroke awareness.
7. Enter data into the Get with the Guidelines program. Retrieve data, analyze and present data to the Performance Improvement Committee making suggestions for improvement as appropriate.
8. Maintains a minimum of 8 hours of stroke education annually.
MINIMUM QUALIFICATIONS: Education: Graduate of an accredited nursing program. Bachelor's Degree in Nursing is required. Minimum Experience: Critical Care Nursing experience is required. Required Licenses/Certifications: California Registered Nurse is required. Required Licenses/Certification: BLS – Basic Life Support Certification issued by the American Heart Association (AHA). Required Licenses/Certification: ACLS – Advanced Cardiac Life Support Certification – issued by AHA. Required Licenses/Certification: NIH Stroke Scale Certification. Preferred Licenses/Certification: CNRN - Certified Neuroscience Registered Nurse within one year of employment.
